Insurance and Payment
Which forms of payment are accepted?
Payment for your session is due in full at the time of service. Cash or personal checks are the preferred methods of payment. Credit cards payments are accepted via Square and may be subject to an additional 3% convenience charge.
What should I ask when I call my insurance company?
When you call your insurance company to inquire about coverage, some important questions to ask about Medical Nutrition Therapy (MNT) coverage are:
-Do I need prior authorization before nutrition services are rendered?
-Is there a limit to the number of visits I am allowed?
-Do I need a referral from my primary care physician for services?
-What are my out-of-pocket expenses?
MNT initial visit CPT (procedure) code is 97802. Follow-up visit CPT code is 97803. The most common ICD-10 code for nutrition services is Z71.3. This information may be helpful when contacting your insurance company to inquire about benefits surrounding nutrition services.
Do you accept health insurance?
At this time, I am a fee-for-service provider and do not bill insurance companies; however, I will provide a "Superbill" upon request which contains a procedure code and diagnosis code for you to submit to your insurance company or flexible spending account for reimbursement.
Insurance and flexible spending or health care spending account coverage for medical nutrition therapy or nutrition counseling varies depending upon the contracted agreement with your employer. Please contact your insurance company and/or FSA/HSA account administrator to determine their policy.
Preparing for Appointments
What should I bring to my appointment?
Please bring your completed paperwork which can be downloaded prior to your appointment from this website under the "Client Forms" tab.
Be sure to also bring a list of current medications and supplements, any recent lab work, a list of your medical providers, and any food logs/journals you may have.
What if I need to cancel or reschedule my appointment?
A minimum advance notification of 24 hours is required for cancellation or rescheduling of all scheduled appointments.
You will be charged a cancellation fee if you fail to comply with this policy.
No Show Policy
If you fail to show up for your appointment and honor your commitment to your session time, regardless of the reason, you are responsible for the full session fee.
